ARTWORK SUBMISSION FORM

- Please provide directions for the placement on your web site of each image submitted.
- A Special Directions box is provided at the end for additional clarification, if needed.
- Leave unused sections blank.
- Maximum of 10 images per submission. You will have the opportunity to add more after submission.

- Visit http://www.sitedesignworks.com/pricelist for Image Addition pricing.
If you have been given a special price quote, please note this in the Special Directions box at the end of the form.

- You may not submit your images online if they are larger than 10MB each, or if the combined image total exceeds 20MB.
If combined total exceeds 20MB, make several smaller submissions or send a CD via regular mail to:

NOTE - Large image submissions can take up to 15 minutes to upload, depending on your connection speed.
Some computers may time out if the upload takes this long. If you are not directed to a confirmation
